Pragmatic Free Trial Meta

Pragmatic Free Trail Meta is an open data platform that enables research into pragmatic trials. It gathers and distributes clean trial data, ratings, and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses that examine the effect of treatment across trials with different levels of pragmatism.

Background



Pragmatic trials are increasingly acknowledged as providing evidence from the real world to support clinical decision-making. The term "pragmatic", however, is not used in a consistent manner and its definition and assessment need further clarification. Pragmatic trials are intended to guide clinical practices and policy decisions rather than confirm a physiological hypothesis or clinical hypothesis. A pragmatic study should strive to be as close as it is to the real-world clinical practice which include the recruitment of participants, setting up, delivery and execution of interventions, determination and analysis outcomes, and primary analyses. This is a major distinction between explanation-based trials, as described by Schwartz and Lellouch1 that are designed to confirm the hypothesis in a more thorough manner.

Trials that are truly pragmatic must not attempt to blind participants or clinicians in order to result in bias in the estimation of the effect of treatment. The trials that are pragmatic should also try to attract patients from a variety of health care settings to ensure that their findings can be applied to the real world.

Finally studies that are pragmatic should focus on outcomes that are important to patients, such as quality of life or functional recovery. This is particularly important in trials that require the use of invasive procedures or could have serious adverse effects. The CRASH trial29 compared a 2-page report with an electronic monitoring system for patients in hospitals with chronic cardiac failure. The catheter trial28 on the other hand, used symptomatic catheter associated urinary tract infection as the primary outcome.

In addition to these aspects pragmatic trials should reduce the procedures for conducting trials and requirements for data collection to reduce costs and time commitments. Furthermore pragmatic trials should strive to make their findings as applicable to real-world clinical practice as possible by making sure that their primary analysis is the intention-to-treat approach (as described in CONSORT extensions for pragmatic trials).

Many RCTs which do not meet the criteria for pragmatism but have features that are in opposition to pragmatism, have been published in journals of different types and incorrectly labeled pragmatic. This can lead to false claims of pragmatism and the use of the term needs to be standardized. The creation of a PRECIS-2 tool that can provide an objective, standardized evaluation of the pragmatic characteristics is a good start.

Methods

In a pragmatic research study the aim is to inform policy or clinical decisions by showing how an intervention could be integrated into routine care in real-world contexts. This is different from explanatory trials, which test hypotheses about the cause-effect connection in idealized situations. In this way, pragmatic trials could have lower internal validity than studies that explain and be more susceptible to biases in their design, analysis, and conduct. Despite these limitations, pragmatic trials may be a valuable source of information for decisions in the context of healthcare.

The PRECIS-2 tool assesses the degree of pragmatism within an RCT by scoring it across 9 domains, ranging from 1 (very explicit) to 5 (very pragmatic). In this study the areas of recruitment, organisation and flexibility in delivery, flexibility in adherence, and follow-up scored high. However, the principal outcome and the method for missing data was scored below the pragmatic limit. This suggests that a trial can be designed with well-thought-out practical features, but without damaging the quality.

It is difficult to determine the level of pragmatism within a specific trial since pragmatism doesn't possess a specific characteristic. Some aspects of a research study can be more pragmatic than other. Furthermore, logistical or protocol modifications made during an experiment can alter its score in pragmatism. Koppenaal and colleagues found that 36% of 89 pragmatic studies were placebo-controlled, or conducted prior to licensing. Most were also single-center. They are not in line with the norm and are only considered pragmatic if the sponsors agree that such trials aren't blinded.

Additionally, a typical feature of pragmatic trials is that researchers attempt to make their findings more valuable by studying subgroups of the trial. This was a problem during the meta-analysis of pragmatic trials because secondary outcomes were not corrected for covariates that differed at baseline.

Schwartz and Lellouch1 created a framework to distinguish between explanation-based trials that support a physiological or clinical hypothesis, and pragmatic trials that inform the selection of appropriate treatments in clinical practice. The framework was comprised of nine domains, each scored on a scale ranging from 1-5, with 1 indicating more lucid and 5 suggesting more pragmatic. The domains included recruitment, setting up, delivery of intervention, flex compliance and primary analysis.

The initial PRECIS tool3 included similar domains and an assessment scale ranging from 1 to 5. Koppenaal and colleagues10 created an adaptation of the assessment, dubbed the Pragmascope which was more user-friendly to use for systematic reviews. They discovered that pragmatic systematic reviews had a higher average scores across all domains but lower scores in the primary analysis domain.

This distinction in the primary analysis domains could be due to the way in which most pragmatic trials approach data. Certain explanatory trials however do not. The overall score was lower for systematic reviews that were pragmatic when the domains on the organization, flexibility of delivery and follow-up were merged.

It is crucial to keep in mind that a pragmatic study should not mean a low-quality trial. In fact, there is increasing numbers of clinical trials that use the word 'pragmatic,' either in their title or abstract (as defined by MEDLINE but which is neither sensitive nor precise). The use of these terms in titles and abstracts may suggest a greater awareness of the importance of pragmatism, but it isn't clear if this is manifested in the content of the articles.

Conclusions

As the importance of real-world evidence becomes increasingly popular, pragmatic trials have gained popularity in research. They are randomized trials that evaluate real-world care alternatives to new treatments that are being developed. They involve patient populations that are more similar to those who receive treatment in regular care. This method has the potential to overcome limitations of observational studies that are prone to biases associated with reliance on volunteers, and the limited availability and coding variability in national registry systems.

Pragmatic trials also have advantages, including the ability to use existing data sources and a greater chance of detecting significant distinctions from traditional trials. However, pragmatic tests may still have limitations which undermine their effectiveness and generalizability. Participation rates in some trials may be lower than expected because of the healthy-volunteering effect, financial incentives, or competition from other research studies. The requirement to recruit participants quickly restricts the sample size and impact of many pragmatic trials. Practical trials aren't always equipped with controls to ensure that observed differences aren't caused by biases in the trial.

The authors of the Pragmatic Free Trial Meta identified RCTs published up to 2022 that self-described as pragmatic. They assessed pragmatism by using the PRECIS-2 tool, which includes the eligibility criteria for domains as well as recruitment, flexibility in adherence to interventions, and follow-up. They discovered that 14 of these trials scored as highly or pragmatic sensible (i.e., scoring 5 or higher) in any one or more of these domains and that the majority of these were single-center.

Studies with high pragmatism scores tend to have broader criteria for eligibility than conventional RCTs. They also include patients from a variety of hospitals. The authors argue that these traits can make the pragmatic trials more relevant and applicable to daily practice, but they do not necessarily guarantee that a trial using a pragmatic approach is completely free of bias. The pragmatism is not a fixed attribute; a pragmatic test that does not have all the characteristics of an explanatory study may still yield valuable and valid results.
